The Practice Guidelines are designed to help parties involved in an appeal. They explain the steps and administrative requirements involved in the processing of an appeal.

Properly completing the correct form helps in processing an appeal quickly.

Notice of Appeal Form

Notice of Representation Form

Notice of Intention to Participate Form

Certificate of Readiness to Proceed (CRP) Form

Application for Extension of Time Form

Request for Transcript Form

There are two ways to complete the forms:

  • You can print a paper copy of the form and complete it manually
or
  • You can complete the information in the PDF Form via computer prior to printing.
*NOTE: You cannot save data typed into these forms. Please print the completed form if you would like a copy for your records.

Because e-mail is not a secure form of communication, do not use e-mail to notify the Appeals Commission of your intention to initiate an appeal, request information, send information about an appeal, update your address or provide other personal information.
